If you're submitting your family photo for the parish photo directory (if you didn't have it taken at the parish hall earlier this year), please download and complete the "Submitted Photo Info" PDF form. You may then attach the completed PDF, along with your photo attachments, below. Or, simply turn in the completed "Submitted Photo Info" form along with your photo to the parish office.
If you are only submitting a group or activity photo for inclusion in the parish directory, there is no need to complete this PDF form - simply attach your digital photo file and provide the description & names using the form below. Thank you!
